PRINCIPAL DUTIES AND JOB
Responsibilities:
Patient
Experience:
Ensures that every patient has a documented and purposeful nurse leader round every day. Rounds by nurse assignment and provides immediate feedback to nurse after rounding.
Validates and holds staff accountable for purposeful hourly rounding.
Unit performance is measured through HCAHPS scores.
Unit Productivity Performance:
Acts as the charge nurse/nurse leader of the unit for each regular shift assigned.
Coordinates unit staffing for shift as described by staffing grid. Prevents overstaffing and over-shift overtime. Justifies on charge nurse staffing sheet any variance to staffing per grid for assigned shift.
Coordinates appropriate staff assignments for shift based upon patient acuity and staff competency.
Documents staffing for shift in the assignment books on each unit.
Gives feedback to the Director and/or House Supervisor on staffing of the unit. Assists with contacting staff for unit coverage as needed.
Explores and implements strategies to reduce productivity costs on the unit.
Human Resources:
Participates in peer interviews for potential new staff.
In collaboration with Director, conducts performance evaluations of staff assigned.
Gives suggestions and feedback to Director for corrective actions or recognition of their staff members.
Clinical and Quality Performance:
Every AM after bed tracking meeting, initiates huddle on the unit to form a team approach to care. Huddle includes a quick discussion of any topics brought forth in the bed tracking meeting, patient flow plan for the shift, staff concerns, patient concerns/needs, IPOC reminders, VTE documentation of interventions, pts at risk for falls, pts with pain not controlled, staff roles during a code blue, etc.
Ensures that staff have complete all required documentation including but not limited to Admission databases, Home medication lists, IPOCS, Vaccine and VTE assessments and interventions, Riskmasters, and medication PPID processes.
Is an active participant in shared governance including unit based counsel and the peer review processes.
Staff Education:
Contributes to the professional development of self, peers, colleagues and others by facilitating a positive orientation experience for our new employees through role modeling and the demonstration of our team oriented environment.
Acts as a super-user on the unit and assists staff with various computer application uses.
Assists staff in competency validation for unit based skills.
Resources:
Ensures staff have the tools and equipment needed to do their job.
Ensures tools and equipment are working properly and notifies the appropriate parties when repair is needed.
Notifies Director of unit needs for next budget planning sessions.
Patient Flow:
Ensures and promotes efficiency through ED throughput initiatives with timely discharges, timely bed assignments, and timely admissions from the ED.
Promotes staff to utilize the one call process consistently.
When holding pts in the ED, works closely with Director, physicians, charge nurses, and house supervisor on discharges and status changes to provide adequate bed availability to the ED. Ensures these processes are continuously followed even if patients are not in hold status.
Other
Responsibilities:
Assesses, develops, implements and evaluates a comprehensive plan of care through a multidisciplinary care team process in conjunction with the patient/family in internal and external settings.
Comprehensively assess patients' goals as well as their biophysical, psychosocial, environmental and discharge planning needs and financial status.
Procures services and advocates on behalf of the patient/family for scarce resources.
Works collaboratively with staff members involved in care of case managed population.
Seeks peer consultation regarding cases that present problems/significant variances.
Acts as a liaison to post-hospital care providers and community health resources.
Manages each patient transition through the system and transfers accountability to the appropriate person/agency upon discharge.
Recognizes and identifies life threatening situations, responding with immediate action according to protocol with no unjustified documented variances.
Demonstrates the skill and judgment necessary to implement physician orders, nursing interventions and procedures as necessary for the care of the patient to provide quality care.
Accurately administers medications, IV fluids and IV medication, blood and blood products according to physician's orders and nursing policy and procedure with no more than three (3) occurrences per year.
Demonstrates ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and nursing policy and procedure with no more than three (3) documented occurrences per year.
Demonstrates knowledge of and administers accurately thrombolytic therapy as per hospital policy and procedure.
Observes, documents and reports patient's response to medications as evidenced in nurse's notes with no more than four (4) omissions per quarter.
Implements standing orders for Code Blue and life threatening dysrhythmia completely, competently, including management of airway, assisting intubation, performing counter shock, administering emergency medications, and directing code activities in the absence of a physician or in conjunction with a physician as evidenced by feedback from physicians, peers and direct observation with no documented concerns.
If applicable, administers and titrates inotropic/vasoactive infusions accurately per policy/procedure and standards.
If applicable, competently cares for a patient requiring hemodynamic monitoring including assisting with catheter insertion as per policy/procedure and accurately interpreting monitor strips and documentation, with less than one-two documented incidents per year as observed by charge nurse and director.
If applicable, competently care for a patient requiring mechanical ventilation as evidenced by functioning every one to two hours, maintaining patient airway, preventing accidental extubation, providing and documenting trach care every shift with no documented concerns.
If applicable, operates cardiac monitoring equipment competently and initiates and maintains continuous cardiac monitoring per unit policy 100% of the time.
If applicable, competently cares for a patient requiring temporary pacing, including assisting with temporary pacemaker insertion, according to policies and procedures as observed by charge nurse, director, feedback from peers with no documented concerns.
Correctly transcribes physician's orders and signs off appropriately after examining orders for clarity, completeness and accuracy with no more than four (4) documented occurrences per year.
Utilizes nursing judgment in implementing physician orders in a safe and conscientious manner, notifying appropriate supervisor immediately of any concerns.
Documents drug allergies 100% of the time upon patient admission.
Monitors lab data and diagnostic tests received on assigned patients.
Documentation includes evidence of appropriate patient/significant other teaching and their understanding of the instruction to assigned patient.

Qualifications
What you will need:
Skill and proficiency in applying highly technical principles, concepts and techniques which are central to the nursing profession and is normally acquired through completion of a BSN and/or MSN program
A minimum of three (3) years of recent experience in an acute care facility successfully demonstrating skills in assessing, planning, implementing, and evaluating patient care required
Experience in unit area specialty in the past two (2) years and one (1) year Management and/or Charge experience preferred
Clinical Pathway/ Clinical Ladder experience preferred.
Current licensure to practice as a Registered Nurse in the state of Texas or in a qualifying compact state
Maintains ACLS (and/or PALS if applicable to unit) certification.
Advanced Certification specific to the unit for which they manage is preferred
